Welcome to the DSS Student Intake/Registration Form!

You can use this form to:

  • Initiate the intake process with DSS
  • Complete the 'Online Intake/Registration Form' **
  • Provide documentation to be reviewed by a DSS staff member
  • Request services for a temporary medical condition, illness, or disability

You will receive a confirmation email once you have successfully completed the Online Intake/Registration Form. 
DSS staff will also be automatically notified of your request. We will review your information within 1-2 days.

Please schedule an appointment with DSS to discuss your accommodation needs:
  - Call DSS at 603-862-2607
  - Email DSS at disability.office@unh.edu
  - In-person at 201 Smith Hall (Monday - Friday from 8am to 4:30pm)
  - 24 to 48 hours after receiving your confirmation email you may utilize the online 'Appointment Tool' in ClockWork

   **Note: When completing the Online Intake Form, students will be asked to upload supporting documentation in an electronic format.
