Welcome to the SAS Student Intake/Registration Form!

The Intake/Registration Process has Two Steps:

1.  Complete the Intake/Registration Form

Please read the information below before you begin the registration process. To begin the process, click the 'Register' button above. You will be asked to login using your UNH username and password.
(Please note: Complete the 'Online Intake/Registration Form' in one sitting and Safari and Internet Explorer 10 have been reported to have some technical issues due to version updates)

You can use this form to:

  • Initiate the intake process with SAS
  • Complete the 'Online Intake/Registration Form' **
  • Provide documentation to be reviewed by a SAS staff member
  • Request services for a temporary medical condition, illness, or disability
  • You will receive a confirmation email once you have successfully completed the Online Intake/Registration Form
  • SAS staff will also be automatically notified of your request. We will review your information within 1-2 days
  • Once the Intake Form is completed, please contact the SAS office to make an appointment for a one-on-one meeting

You will be asked to provide the following information and we suggest you gather it before you log in and start the intake process:

  • Basic student information
  • Identify the nature of your disability
  • Describe how your disability or temporary medical condition impacts learning
  • Accommodations you are requesting
  • Accommodations from your previous institution (High School, College, University)
  • Contact info from your previous institution (High School, College, University)
  • Contact info for Clinician, Primary Care Physician, or Therapist (Third party who can speak to the functional impact of your disability)
  • There are 5 slots to upload digital documentation, if you need to upload more items than that please send them to our office. 

2.  Meet with a SAS staff member

After you have completed the intake/registration form, it is the responsibility of the student to reach out to our office to schedule an appointment with SAS to discuss your accommodation needs:

  • Call SAS at 603-862-2607
  • Email SAS at SAS.Office@unh.edu
  • In-person at 201 Smith Hall (Monday - Friday from 8am to 4:30pm)
  • 24 to 48 hours after receiving your confirmation email you may utilize the online Appointment Tool in ClockWork


When completing the Online Intake Form, students will be asked to upload supporting documentation in an electronic format.

  • If you are unable to upload documentation, please be prepared to provide it when you meet with SAS Staff.
